To determine if you need to provide a medical certificate, you must complete the Health Questionnaire.
We invite you to return to the form after completing the questionnaire.
To determine if your child needs to provide a medical certificate, you must complete the Minor's Health Questionnaire.
Please submit your certificate of completion of a health questionnaire (download template here) confirming that you have answered "no" to all the sections.
You must also provide a medical certificate dated less than 6 mouths ago and mentioning "fit to practice skiing or sport in competition".

If you wish to start in the elite or 1st line, you will be asked to indicate your best results over the last 2 years (2022-23 or 2023-24 season).
By checking the following case you confirm that you are the legal representative and authorize the child to participate in this event organized by Haut-Jura Ski.
You have the choice between "full meal on site" and "full meal to go".
